Interdisciplinary Coordinated Care of the Child with Chronic Illness

Join health care students in pediatrics, nursing, pharmacy, social work, nutrition, child and family studies, and respiratory therapy as they learn about issues related to caring for children with chronic illness. You will have access to practicing physicians, nurses, respiratory care practitioners, social worker, nutritionist, and pharmacist.
This online course uses pediatric pulmonary care as a model for interdisciplinary team management for children with chronic illness. In the course you'll broaden your concept of health care by sharing the perspective of other health care professionals. You’ll have an opportunity to function as part of an interdisciplinary team by learning, collaborating and problem-solving together to complete course assignments. You’ll interact with a variety of different faculty for each lesson with continuity provided by the course instructor.

Course Section 1: Interdisciplinary Teams
Discover the importance of good team function and how interdisciplinary teams can enhance the healthcare for children with special healthcare needs (CYSHCN).
Learn about the course and how to navigate it. Meet your fellow students and course faculty. This section introduces team function, which is a central concept to interdisciplinary care. Learn how concepts of family centered care, confidentiality, and team splitting can impact health care teams.
Course Section 2: Family Perspectives of Transitions
Discover a Pediatric Pulmonary Center (PPC) transition model that can be applied to transitions of CYSHCN such as discharge home and death. Learn about these transitions from a family’s perspective.
Learn about a model of transition that can be used to support families through a variety of transitions. Transitions throughout the life span will be examined. Examples such as the birth of a child with special needs, discharge from hospital to home, transition to adult care, and death will be more thoroughly explored. Two families will share their specific experiences and perspectives of some of these transitions.
Course Section 3: Patient Provider Relationships
Discover how issues such as cultural diversity and adherence can affect the relationship of the families of CYSHCN and the interdisciplinary health care team.
Learn about providing health care to a culturally diverse population. Being aware of your own cultural biases is the first step in becoming culturally competent. Working effectively with interpreters is another important skill. Understanding factors affecting adherence, methods to assess and interventions to improve adherence can have a significant impact on the management of chronic illness.
Course Section 4: Provision of Healthcare
Discover the impact that health care delivery systems have on families of CYSHCN and how legislative advocacy can improve care for this population.
Learn about the population of CYSHCN and the financial resources available to fund health care for children. The Maternal Child Health Bureau (MCHB) has numerous programs that affect the health care delivery systems for this population. Legislative advocacy is one mechanism that may be used to address inadequacies in the provision of health care services.
