Learning Activities

Before you begin, please review the content on transition and the Transition Model. These learning activities will allow you to explore the case of Sally in greater detail and apply the Transition Model to this case.

Activity 1: The Inside Story

Sally, Teri and their respiratory therapist, Dan, approach this situation from totally different perspectives. This activity will enable you to gain insight into the perspectives and thoughts of each of these individuals.

In this activity you will use the Transition Model to analyze the management of Sally’s asthma as she transitioned to school. Applying the Transition Model to health care related transitions can help to plan for anticipated transitions. Review this model before proceeding with this activity.

Activity 3: Exploring Interventions

In this activity you will be participating in a simulated team meeting with your colleagues to explore possible interventions that could have made Sally’s transition to school smoother.
